Other CPhA Initiatives and Resources: Home Care
Including pharmacists in community-based home care teams is a key opportunity for pharmacists to play an expanded role beyond the dispensary. The First Minister's Accord on Health Care Renewal has set core requirements for a national home care program as a priority.
CPhA has produced a position paper and we are working with provincial pharmacy advocacy organizations to get the message heard and accepted: pharmacists' services, including medication reviews, must be part of the core set of home care services and payment models must be developed.
We are completing a project, funded by Health Canada's Population Health Fund, to study the role and duties of home support workers, develop training programs, and identify specific pharmacists' services to support home care workers in optimizing medication use in seniors receiving home care.
